Repeat Prescriptions

Please email all repeat prescription requests to Prescriptions.W97034@Wales.NHS.UK or drop a note into the prescriptions box to the left of our front door.  Alternatively you can complete the below form.

We DO NOT accept prescription request via telephone. We require an audit trail for all requests. 

We require 48 -72 hours to complete a repeat prescription, these can then be sent to your elected local pharmacy. Please allow sufficient time for both the surgery and your local pharmacy to complete your request before you require your medication. 

If you have never had a medication from the surgery you will need to provide proof that you are on this medication, such as a picture of a box (with you name, dose and date clearly on display) a repeat prescription slip from your previous surgery or a letter from an NHS doctor. 

Please be mindful, if you request your medication too early this may be rejected unless you have provided a reason for the early request (such as being on holiday when your medication is due).

 

 

Shared Care with Private Organisations

The practice cannot prescribe on behalf of private providers nor continue to supply prescriptions which have previously been supplied by the private provider unless this is a medication which a GP would normally prescribe and the prescription is in line with local health board guidelines. We are not permitted to have shared care protocols with private providers.

Private consultants wishing to transfer patients to NHS care should directly refer their patient to the appropriate NHS service who will reassess and, if appropriate, take over their treatment. At the point that their care has been taken over by an NHS service, a request may be made by the NHS service to share prescribing responsibility with the practice. Our practice pharmacist will review and manage all such requests in line with the local health board guidelines.
