REGISTERING WITH ROATH HOUSE SURGERY

Roath House Surgery can only accept patients from within our practice area. Please see image below, if it is not clear whether you would be in our area please email us before completing any registration forms. 

We require the GP registration form and either the Registration Questionnaire or the Under 5's Registration Questionnaire for every patient. Without this we will not be able to process your registration.

If you are a student you can register with us via campus doctor.
